What does my prescription need to include?
Service hours
- Monday—Friday 9am–6pm
- Saturday 10am—6pm
- Sunday 10am—6pm
We'll need a copy or photo of your prescription that shows the following information;
- Full name, address and D.O.B
- Your optician's details with a signature
- The date your prescription was issued
- Prescription expiry date
- and of course, your prescription
If your prescription is missing some of these details, your optometrist will be able to provide you with a copy that does.
